Volunteer Application Form

BY SUBMITTING THIS APPLICATION I AGREE NOT TO HOLD SMILEFEST AND/OR STAFF RESPONSIBLE OR LIABLE FOR ACCIDENTS, LOSS, THEFT, OR CLAIMS RESULTING FROM MY PARTICIPATION.
I understand that my application will not be processed until SmileFest Volunteer Coordinator has received my volunteer deposit and volunteer application fee. This volunteer deposit is $90 will be held until the final day of my scheduled task. If all my scheduled tasks are completed successfully, I may pick up my MONEY ORDER from the Volunteer Coordinator before leaving the festival grounds. The volunteer application fee is a non-refundable fee of $15. The deposit of $90 should be mailed in the form of a MONEY ORDER made out to “CASH”. The application fee of $15 should be mailed on a SEPARATE MONEY ORDER made out to “CASH”. After filling out this application, you may submit it online or mail it to address below. All money orders should also be mailed to the address below:

*For accurate scheduling, it is recommended that you send your applications together.

Shift Availability

Please check the days and times you are available to work (check ALL days and times you are available. Also note we will need volunteers to work pre and post festival so be sure to let us know if you would like to work before or after):

Work Preference

Please indicate preferred areas to work. Every consideration will be given to your preferences, however Smilefest schedules volunteers as needed.

The different areas include: Parking, site-operations (mainly set-up/breakdown), hospitality, administration, security, traffic control, trash/recycling, and more.

Note: Please make baby-sitting arrangements prior to your assigned shift.
